Read about Hotfixes here. As a practice I only apply hotfixes to a client's install if they report a problem that is fixed in a hotfix. Hotfixes can be easy to install and can be hard, all depends on how you customized your install. By default, hotfixes don't cause breaking API changes and are fairly simple to install without causing problems.
