Display alternative form based on Scope

I have a page type that I'm using for two slightly different purposes. The standard form works well for the primary use case, but the field captions are a little non-intuitive.

I have created a scope based on page type and created an alternative form with different field captions based on my other use case, but I can't see how to configure Kentico so that this alternative form is used on this page.

I Am afraid but there is no such a setting available. The alternative forms are used automatically based on the reserved code name. In this case I would maybe rather recommend creating a fully custom ASPX page with the form and set it for the particular page types as a new and editing page (sample screenshot).
