XML Sitemap only contains 2 URLs

Question title is exactly the issue. I have a sitemap.xml being generated but it only contains the home page and one other location.

What can I do to ensure these pages are all going to my sitemap?

What version of Kentico are you using and also what is the XML sitemap configuration? What web part is used, what are the settings set in the URLs and SEO section? Is the runAllManagedModulesForAllRequests attribute present in the web.config file at the right place? What transformation is used to generate the sitemap?
