Kentico 7 to 8 Upgrade - Forms Issue

After updating our devserver from kentico 7 to 8 we noticed a couple of issues with Forms. 1st issue is Phone Number fields are appended with "First three digitsSecond three digitsLast four digits" and the radio button field does not display correctly. The options are pushed outside of its container (down) and the radio buttons do not even appear. Thank you.

I believe the form control changed the layout. If I remember right you can specify a different layout for the control. The other option would be to use a standard textbox now and use the fancy new macro conditions to validate a regular phone number. That would be the quickest, simplest fix IMHO.

Having the "First three digitsSecond three digitsLast four digits" text should be configurable.
