Where is the checkbox control displaying? Form? Page type? Custom module class? Need more description than what you've provided. Maybe a screenshot of the configuration of the checkbox control along with what final display, anything is better than what you've provided.

I'm not seeing 3 checkboxes being displayed at all. On the form go to the Fields tab and select the actual field name and display the values for the field with the problem.

Hi Brenden Kehren,

I am just using Checkbox control not multi checkbox, I need only one checkbox to be displayed. for some reason it is displaying 3 instead of one.

It is adding one extra div when I see the source code.
