Form Has Vanished

Hi DevNet! Got a problem. Kentico v9. The form on this page is no longer displaying and I can see no evidence of anything being modified in Kentico. We were receiving data from the form as late as two days ago. This form is a super-important business driver, so we need this form back up ASAP.

Note that I did not create/implement the form. Just wanted to put that out there. In the Kentico admin interface, I cannot see where/how this form is being written to the page. It's not like there's an online form web part inside a zone. Any thoughts on this would be welcomed, too.

Here's what I've done:

  1. The form. I do see the form in the Forms application in Kentico. The form has not been modified in over two years.
  2. The Web Part. There is a Web Part in the system with the same name as the form. When editing the Web Part, in the Usage tab, Kentico tells me that this Web Part isn't being used anywhere in the system.
  3. The Page Type. Kentico tells me in the General tab that the page is using a Page Type with the same name as the form. This Page Type was last modified nine months ago.
  4. Try something new. I created a new page using the same Page Type and otherwise matched the existing page with the missing form. Nothing.
  5. Event log. I went to look through events from two days ago, but the logs only go back one day (Dec 14). So that's no help.

Can you all help me think through this issue? Why is the form suddenly not displaying?

Has the page got versioning? It's strange if nobody has modified the page for 9 months.

Can you paste some screen shots of the template area/design tab?

Is this the form that appears from the right (Schedule Your FREE In-Home Assessment)? Or a form on the page content area itself?

Thanks, Ilesh. I recreated the form and added it to the offending page. It's working for now. But I still don't know why the form would have disappeared. I appreciate your fast responses! Thanks again.
