Form attachments have gone missing

Hi,

I'm using Kentico v12 with Portal and have been experiencing strange issues with Page attachments disappearing.

I have a custom page type which uses a field "MenuItemTeaserImage" and the Direct uploader Form control to enable editors to upload images (this all appears to be standard).

Somehow when editors save/publish the image is being saved but the file goes missing. Image Text

When I click the URL I a URL like this which does not work (image missing): www.mykenticosite.com/cms/getattachment/cc96c6d0-f2c1-4564-85b8-5ce13ae350f3/Advocacy.jpg.aspx?versionhistoryid=144712&chset=7e61e915-4fb4-4f99-b508-ed26182a6eae

However if I remove the query parameters for versionhistory then it works correctly. For example: www.mykenticosite.com/cms/getattachment/cc96c6d0-f2c1-4564-85b8-5ce13ae350f3/Advocacy.jpg.aspx

Any idea's on why this is happening or encountered a similar issue?
