I think I have found the issue. It has to do with the website I imported from 4.0 to 4.1.
I installed 4.1, added my custom code and everything worked as expected.
Then I imported my website and the New Attachment link on the attachment page is no longer active.
This has been experienced with a Page Menu Item and an Article document. I have tested this on existing pages and new pages created under ver. 4.1.

Hi,

using the export/import for upgrading the web sites to newer version is not a recommended way, because importing older web site you can overwrite the new version files back to the old files. Therefore you have to import only your custom objects and documents. It is possible that some objects were reverted back to the 4.0 version state.
