Intranet Template - Default Form Contents
While trying to get something to work, I inadvertently cleared the contents of one of the fields within a form; unfortunately, I have thus far been unable to restore the correct contents and so, the form is not currently working.

The site is based on the Intranet Template, and the form is for the Department Forum page Template.

If someone could provide the default value for that field, I would be very grateful.

Do you have versioning enabled? If so you can simply go to that page's properties page and look at the versions and roll it back.

RE:Intranet Template - Default Form Contents
To the best of my knowledge, I do have Versioning enabled as I have noticed it on certain Web Parts; unfortunately, I can not seem to find any Versioning options either for the Forum Web Part, or for the Page itself.

Below is a screenshot of the Page having been selected within CMD Desk:

This problem has now been resolved.

For others' reference, the default value for 'Group name' field is "Department_{%cmscontext.currentdepartment.nodeguid%}".

RE:Intranet Template - Default Form Contents
It doesn't appear you have versioning enabled for page templates. Since web parts are part of the page template, you'd need to enable versioning for page templates in CMS Site Manager>Settings>Versioning & Synchronization>Object Versioning and select Use object versioning for Page Templates. The versioning for the web part is strictly for changes made to the base web part and will not matter what page it is on. Glad to hear you got your issue resolved.

Enabling Versioning would be useful, but it seems that I already have that option enabled:

Any ideas?
