All other aspects seemed to work just fine. Make sure to post back with your results, I'd be interested to hear how it works for you.

UniformJS seemed to work but couldn't do a thing with the check-boxes and the radio buttons so I've removed it for now.

I suppose you could create your own radio button or checkbox list by looping through your results and simply creating a new object an setting a group/name so it stays in the same "group". Not ideal but would work.
