Form contact
Hi all,

In Form Contact, I have property: name, email, type contact

have 2 type contact: FAQ and Buy product
When I checked in FAQ, show Title, Content
Check in Buy product, show ProductID, Quantity, ProductName, Note.

How can I do this?

This can be done easily in the version 7 of Kentico CMS and here is a description for creating these kind of forms in v7:
