Are you working as an Admin on the site, if not, you might have to check to see which roles you are in. The UI has a lot of customization you can add based on roles and the WISIWYG editor has a lot of custom UI properties as well. I believe setting visibility is one of them. If that's not the case, then it could be a number of other things like missing a .dll or problems with your browser, conflicting add-ins on your browser, etc.

Could you please navigate to Site Manager / Development / Document types / edit given document type / Form tab and let me know if custom layout is used? If so, please make sure that it contains $$input:<columnCodeName>$$ macro. Or you can click “Generate table layout” and generate new one. Thank you.
