Where are the 48x48 document type icons used?
I had noticed before that for each document type there are 2 icons, the small one is visible in a lot of places, but the larger 48x48 one I haven't seen (yet).

Where are the large icons visible?

Hello,

Large icons are shown in the Media Dialogs (when clicking e.g. to Insert/Edit Link button of WYSIWYG Editor). You must switch to Tiles or Thumbnails previews to see them.
