In the free version on kentico:

what counts as a document, do comments on a document count, do items in the media library count?

is there somewhere you can see the number of documents so you can keep track of how close to 1000 you are?

Hello,

The count of documents is measured by the count of entries in the CMS_Document table in the database. Corrently there is regrettably no counter for this in the system implemented.

what is the "rule", which items get stored in the CMS_Document table? As I need to be able to plan for this site

Hello,

If you check the mentioned table, you can see, that there are documents and their language versions. That means, that everytime you add a new document into the content tree or create a new language version of a document, which is already there in an another elanguage, an entry is added into the database.
